- 4 TBS unsalted butter plus more for casserole dish
- 3 lices white bread crusts removed, chopped into ¼- to l/2-inch pieces
- 2 and ¾ cups milk
- ¼ cup all-purpose flour
- 1 teaspoon coarse salt plus more for water
- ⅛ teaspoon ground nutmeg
- ⅛ teaspoon freshly ground black pepper
- ⅛ teaspoon cayenne pepper
- 2 ¼ sharp white Cheddar cheese, grated
- 1 cup Gruyère, grated (or ⅔ cups grated Pecorino Romano cheese)
- ½ pound elbow macaroni or other spiral noodle
